This system allows you to make your first appointment without relying on my limited phone availability.

Scheduling your evaluation appointment

  1. You can self-schedule your evaluation appointment through my Patient Portal.

  2. Please monitor your email after self-scheduling. I will send you documents that need to be completed at least 48 hours prior to your appointment. Allow about an hour to complete these forms.

If you would like to speak briefly on the phone prior to scheduling, you can schedule a call.
